In short

Cancellation cost is the production lost to late cancellations you could not refill. Multiply late cancellations per week by the share you fail to refill, then by average appointment value and working weeks, to get the annual figure. Practices with a live short-notice list refill 50–70% of late cancellations in most markets; those without one typically refill under a third. Because the chair, the staff and the rent are already paid for, most of the lost production is lost margin.

How this is calculated

  1. Unfilled per week = cancellations × (1 − refill rate).
  2. Annual cost = unfilled × value × weeks.
  3. This is lost production, not lost profit. If fixed costs are already covered, most of it is lost margin.

Frequently asked questions

How do I refill cancellations faster?

Keep a live short-notice list of patients who want an earlier appointment and text it the moment a slot opens. Practices that do this consistently refill 50–70% of late cancellations. Moving unscheduled treatment patients into the gap works as well, since the diagnosis is already done.

What counts as a late cancellation?

Any cancellation inside the notice period you would need to refill the slot, commonly 24 or 48 hours. Set the definition once, tell patients at booking, and record it consistently in your software so the count is reliable.

What data do I need from my practice software?

A cancellation report showing cancellations by notice given, plus whether the slot was subsequently filled. If your system does not record refills, have reception tally them for a month; the refill rate is the number that changes most with a good process.

Should I charge a cancellation fee?

A fee is a deterrent rather than a revenue source, and it works best when it is explained at booking and applied consistently. Many practices find that a clear policy plus a strong refill process cuts the cost far more than the fee itself ever recovers.

How often should I measure this?

Monthly, tracking both the number of late cancellations and the refill rate. If cancellations rise, look at the reminder sequence; if the refill rate falls, look at the short-notice list.
